End of life facilitators

About the team

End of Life Facilitators work closely with residents, families, care home staff and GPs, to ensure each resident receives high quality care to meet their needs and to avoid unnecessary hospital admissions where possible. The team offers support to Residential and Nursing Homes staff, through provision of training, education, symptom management, completion of Fast Tracks, liaising with families and carers.
