The Practice Nurse team is a new, innovative service set up and will be working with GP practices within Primary Care Networks to provide a service to adults who require additional support with their mental health, including dementia. The team are based mainly within the GP surgery facilities.
The key principles of the service are:
- Working with GPs in primary care to provide physical health checks and review mental health care plans for individuals on the SMI (serious mental illness) register.
- Provision of annual comprehensive physical health checks and care plans for individuals on the SMI register
- Provision of annual review for people living with dementia
- Provision of support and sign posting to other services including the Community Mental Health Teams and community-based services as assessed as appropriate
- Provision of advice, guidance and training for primary care providers
- Working collaboratively with local mental health services and the KMPT Primary Care Mental Health Service, maximising effectiveness and avoiding duplication of care
The team is made up of 3 Primary Care Nurses and are supported by an Assistant Practitioner.
