Long-term conditions or chronic diseases are conditions for which there is currently no cure, and which are managed with drugs and other treatment, for example: chronic obstructive pulmonary disease and other respiratory conditions, diabetes and cardiac conditions affecting the heart.

The Long Term Conditions Teams provide specialist advice and interventions to support patients and other health care professionals and agencies.

They provide direct advice and support in complex patients with long term health care conditions.  The aim of each service is to keep patients well, optimising quality of life and preventing unnecessary hospital admissions where possible.
