Neighbourhood nursing
About the team
The neighbourhood nursing teams provide nursing care for patients, aged 18 and over who are housebound within the Medway area, delivering a range of personalised care and treatment to those with complex and long term conditions, supporting people to stay safe and well at home. A housebound patient is defined as ‘a patient to whom the person providing nursing care would normally offer home visits as this is the only practical means of enabling the patient to access the service ‘face-to-face’. A patient is not housebound if s/he is able to leave their home environment with minimal assistance, for example unassisted/assisted visits to a hairdresser or supermarket.
