The TICC (Transforming Integrated Care in the Community) Final Conference was held on 14th October 2022.  Partners from the UK, France and Belgium shared their experiences and learning, as well as reflecting on placing humanity at the heart of our services as the project’s objective has been to implement and evaluate a new organisational model of domiciliary care services. Over the past 4 years partners have been creating systemic changes in health and social care by implementing this model.

This model inspired by the Dutch Buurtzorg system has evidenced through implementing improved patient outcomes and satisfaction, happier nurses/increased retention and demonstrable financial benefit to the wider health and social care economies.
