Medway Community Healthcare (MCH) is delighted to announce that a systems approach to Single Handed Care training to reduce delays in hospital discharges for those waiting for a package of care has been shortlisted for the Community Care Initiative of the Year at this year’s HSJ Patient Safety Awards which recognises safety, culture and positive experience in patient care, celebrating its worthy finalists on a national scale.

The HSJ Patient Safety Awards acknowledges the hard-working teams and individuals across the UK who are continually striving to deliver improved patient care.

Following the thorough judging process, MCH was included in the 206 organisations, projects and individuals that made the final shortlist amid 415 entries logged for the 2024 awards programme.

Single Handed Care is a system wide training programme to enhance moving and handling skills in clinicians involved in hospital discharge and to build confidence to ”prescribe” optimal carer handling.

The aim of the training is to reduce delays in hospital discharges for those waiting on packages of care that would have traditionally needed more than one carer and support community provider capacity.

The initiative involved integrated training sessions with clinicians from across the system including acute, community health, adult social and private care providers, working alongside partnership commissioning for both community and equipment provision and equipment providers.

Diane Lane, Head of Service Intermediate Care at MCH said: “It is fantastic that our team have been recognised for their invaluable contribution to this project and the difference that it makes to not only the lives of those that we support, but also by supporting integrated working across our system.

“It is such a boost to all those involved - we’re delighted to have been recognised on this prestigious shortlist.”

The Awards are a highlight of the HSJ’s annual 2-day Patient Safety Congress (taking place on 16 & 17 September), where delegates working at the forefront of safety, quality and clinical excellence come together to join in on open and honest discussions about the current reality of safety.
